How to register as a patient

We welcome new patients to the Heathbridge Practice.

How to register

To register with the practice, you need to be a full-time resident within our catchment area - see map opposite. Please check with reception if you are uncertain.

All patients over the age of 5 will be asked to make an appointment with our health care assistant for a health check, and to get details of your medical history.

Children under 5 will be asked to make an appointment with one of our practice nurses. Please bring all information about the immunisations your child has had.

You will need the following:

  • GMS1 form download here or collect from surgery (Please ensure you complete all sections or the Health Authority cannot process your application.)
  • Proof of identity (eg passport, driving licence)
  • Proof of address (eg recent utility bill, bank statement, rental agreement)
  • NHS number if possible
  • Details of any medication you are on
  • Children under 5 - immunisation record

We will be unable to process your registraton application if you fail to bring all of these to your appointment.

Are you entitled to register as an NHS patient?

You must be normally resident in the UK. This means that you must have lived here, or intend to live here, for at least 6 months. You can still see us as a private patient if you are not eligible for NHS care.
