Recurring & Additional Sick Note

If you have already been issued a Sick Note (Fit Note) for this illness, you may not need to be seen anymore.  Instead you may just need to be reissued an additional Sick Note.   In this case please complete the form below.  We will contact you to let you know when you can collect your Sick / Fit Note or we may contact you to arrange an appointment.
