REGISTRATION PROCESS

HOW TO REGISTER


To register with the practice, you need to be a full-time resident within our catchment area - see map. Please check with reception if you are uncertain. All patients over the age of 40 will be asked to make an appointment with our health care assistant for a health check, and to get details of your medical history. Children under 7 will be asked to make an appointment with one of our practice nurses. Please bring all information about the immunisations your child has had.




REQUIREMENTS


Complete our online registration form or can collect a form from the surgery. (Please ensure you complete all sections or the Health Authority cannot process your application) Proof of identity (eg passport, driving licence) Proof of address (eg recent utility bill, bank statement, rental agreement) As part of our registration process we encouarge all new patients to provide the above documents. However if you are unable to at this time, we will still process your registration and you will still be able to access our services. NHS number if possible Details of any medication you are on Children under 5 - immunisation record Registering as a regular patient: You must be normally resident in the UK. This means that you must intend to live here for at least 6 months.
