How to Make Medical and Admin Requests Online 

Since 2019, Heathbridge Practice has been using an online triage form to help patients get in touch with us more easily during our normal opening hours.

This form allows you to contact the practice for both medical and administrative issues. Just answer a few simple questions, and we’ll respond as soon as possible.

Frequently Asked Questions

What Can I Use the Form For?

You can use the form for most of the things you would normally call or visit the practice about, including:

  • Medical concerns you want to discuss with a doctor or nurse
  • Repeat prescriptions
  • Fit (sick) notes
  • Test results
  • Referrals
  • Letters and other admin requests

If your issue is urgent, please call the practice directly.
If the practice is closed, contact NHS 111 for urgent help or 999 in an emergency.
Please note: Forms are only read and processed during practice opening hours (Monday to Friday).

What Questions Will I Be Asked?

  • For medical issues, you’ll be asked to describe your symptoms, how long they’ve been going on, and what your main concerns are.
  • For admin requests, you can select from a list of options such as test results, prescriptions, sick notes, referrals, or general enquiries.

What Information Will I Need?

To complete the form, you’ll need to provide:

  • Your full name
  • Date of birth
  • Postcode
  • Gender
  • Contact number

You’ll also have the option to verify your identity using a code sent to your mobile phone, which helps our team access your records more quickly.

How Long Does It Take?

The form takes just a few minutes to complete. If you have any difficulty using it, please call the practice, and a member of our team will be happy to assist you.
