Devices and data informed consent statement

The organisation helping you is working with a charity called Good Things Foundation to support people with free data and/or devices.

When you receive your data and/or device we need to collect the following information about you:

• The type of support you have received (device/data)
• Your name and contact details
• Whether you consent to be contacted for research and/or quality checking processes

The organisation helping you will:

• Ask if you are happy to take part, and answer any questions you may have
• Read this information out to you and explain your data privacy rights
• Check you give your consent freely
• Enter the information about you, accurately, on your behalf

If you consent to be contacted, Good Things Foundation or its research partners may also contact you by phone, email or text message and ask you some more information about yourself and the support you received.
