We are pleased to invite registered patients from Heathbridge Practice, Tudor Lodge Health Centre, Mayfield Surgery, and Chartfield Surgery to attend our upcoming PRIME Patient Group Meeting.

- Parking: Not available on site
- Format: In-person only (no online attendance).
- Light refreshments will be provided
- Meeting materials and the agenda will be shared digitally via our website to reduce printing and administrative workload.
- This event is open only to registered patients of the practices listed above.
- AI tools will be used for minute-taking.
- Photographs may be taken during the event. If you prefer not to be photographed, please indicate this on the attendance form.
